Querying the Hivemind: Skiffying the Proust Questionnaire

For the ConDFW newsletter, I'd like to create a SF/F/H-related version of the Proust Questionnaire to ask various of our panelists. Preferably with a focus on the answerer's creative works (mostly writers, some artists and editors among them), and questions that are open-ended to allow them to answer with as little or as much as they'd like. And about 1/3-1/2 as long as the original on the Wikipedia page!

Any ideas?
